Form 1095-C (Employer-Provided Health Insurance Offer and Coverage) is now available.

Employees who were eligible for employer sponsored health coverage during the prior calendar year may receive a form 1095-C for their records.

This form provides information about the health coverage that was offered and is not required to be filed with your tax return, but should be retained with your tax documents.
